General Requirements

The following is a list of requirements for the 2022/2023 Enrollment Period. All the Forms listed below are found at our Main Office and Online.
  1. Students must be: 4 yrs old for Pre-Kinder, 5 yrs old for Kidergarten and 6 yrs old for 1st Grade by October 31st of the current year.
  2. Students applying for 2nd to 11th Grade must have a minimum 2.50 GPA (Call the office for further assistance.)
  3. An admission test is required from Kindergarten to 11th Grade.
  4. We will make Advance Placement Recommendations for Spanish and Math during the Summer Break based on the 11th grade GPA.
  5. Parents should make sure all Enrollment Forms and Documents are completely filled out and turned in on a timely manner. 
  6. Enrollment Fee Payments will not be processed until all forms and documents have been delivered to the school. 
  7. All Parents should be aware that Enrollment Fees are non-refundable.

Maternal and PPK Requirements

  • Enrollment Form
  • Birth Certificate (Official Copy)
  • Immunization Record (Updated Immunization Form P-VAC). It is required by law that all Immunization Records are up to date. Students will not be able to start the school year without this requirement. We will need the original green document.)
  • Medical History Form
  • Permission to Administer Medication Form
  • Medical Certificate - must be filled out and signed by medical provider.
  • 2x2 Photo
  • Permission to use Social Security Number for official school business.
  • Socio Economic Study
  • School's Promissory Note
  • Financial Responsibility Contract
  • Pick up Authorization Form (PPK to 2nd grade only. You will need to provide a 2x2 photo of the person(s) authorized to pick up your child.)
  • SPBS will not accept incomplete admission forms.

PK thru 12th Grade Requirements

  • Enrollment Form
  • Admission Test (Kinder - 11th Grade)
  • Birth Certificate (Official Copy)

  • Medical History Form
  • Permission to Administer Medication Form

  • Permission to use and share Social Security Number for official school business.

  • School's Promissory Note
  • Financial Responsibility Contract
  • Official School Transcript
  • Letter of Recommendation from previous school (1st grade thru 11th grade only). (Must be an official letter provided by the school.)
  • Video and Photo Authorization
  • Authorization for the monitored use of computers and internet.
  • Pick up Authorization Form (PPK to 2nd grade only). You will need to provide a 2x2 photo of the person(s) authorized to pick up your child.)
  • Financial References from Previous School (Provide only if the student was enrolled in a private school.)
  • Drug Testing Results (9th - 12th Grade students)
  • ​SPBS will not accept incomplete admission forms.
Saint Patrick's Bilingual School is not an institution that specializes in special education. The school does not have a special education program.
